Description
The combination of Telmisartan 40 mg & Hydrochlorothiazide 12.5 mg tablets is a fixed-dose combination of two antihypertensive drugs marketed by Pharvax Biosciences. This clinically well-proven combination comprises Telmisartan, which is among the most long-acting and cardio-protective Angiotensin II receptor blockers (ARBs), along with Hydrochlorothiazide (HCT), which is the most widely prescribed thiazide diuretic. The combination drug targets both the pathophysiological pathways of hypertension, viz., Renin- Angiotensin- Aldosterone System (RAAS) activation (Telmisartan) & volume-based hypertension due to Sodium and Water retention (Hydrochlorothiazide), which results in better & synergistic lowering of blood pressure by the two agents together, wherein the Sodium loss caused by Hydrochlorothiazide further enhances the RAAS blocking action of Telmisartan.
Uses and Indications
Telmisartan 40 mg + Hydrochlorothiazide 12.5 mg Tablets are prescribed by cardiologists, nephrologists, diabetologists, and general physicians for:
- Essential hypertension – Patients not responding to BP targets on Telmisartan 40 mg or Hydrochlorothiazide alone; combining HCT with ARB is a guideline-recommended and highly potent strategy
- Volume-dependent or salt-sensitive hypertension – Hydrochlorothiazide is a potent drug for salt-sensitive hypertension (common in the elderly population, African race, and patients with high salt intake)
- Hypertension in type 2 diabetes mellitus – The antidiabetic effects of Telmisartan and its renoprotective effect in diabetic nephropathy; Hydrochlorothiazide is well tolerated in the dose of 12.5 mg
- Hypertension in the presence of associated cardiovascular risks – 24-hour BP reduction by Telmisartan reduces morning surge-associated risk of cardiovascular events; established in the ONTARGET and TRANSCEND trials
- Hypertension in the presence of mild to moderate chronic kidney disease – Renoprotective effect of Telmisartan by RAAS inhibition decreases proteinuria and slows the progression of CKD; HCT is used in managing the volume
- Hypertension in elderly population – Once daily formulation of Telmisartan and low dose of HCT makes the combination well-tolerated in elderly patients
- Step-up from ARB monotherapy in hypertension – Low dose of HCT with Telmisartan further lowers the BP by 6-8 mmHg systolic without increasing ARB dose
Side Effects
Telmisartan + Hydrochlorothiazide 12.5 mg is generally well tolerated. The low HCT dose significantly reduces the metabolic side effect burden compared to higher thiazide doses. Reported side effects include:
- Light-headedness/ dizziness — especially upon first arising (orthostatic hypotension); more common during early weeks of treatment or among volume-depleted patients
- Hypokalemia (low potassium) — due to hydrochlorothiazide; risk is very small at 12.5 mg dose, but it needs monitoring; can lead to muscle cramps and weakness
- Increase in serum uric acid levels — hydrochlorothiazide reduces uric acid excretion in kidneys; can trigger gout in predisposed patients
- Increased blood glucose or HbA1c levels — due to thiazides; more of a concern at high doses; no significant increase at 12.5 mg
- Increase in serum creatinine — telmisartan-related; ARB drug class side effect; minor increase that stabilises; need for investigation of bilateral renal artery stenosis if increase is significant
- Hyperkalemia — telmisartan-related suppression of aldosterone; very low risk when combined with HCT but increases when other K+-sparing drugs are added
Precautions and Warnings
- Monitor blood pressure regularly — target < 130/80 mmHg in most adults; < 140/90 in elderly and selected patients per current Indian and international guidelines
- Monitor serum electrolytes (potassium, sodium), serum creatinine, uric acid, and blood glucose at baseline and periodically throughout therapy
- Use with caution in patients with bilateral renal artery stenosis or a solitary kidney — ARBs can precipitate acute kidney injury in these patients
- Inform the prescribing doctor of all concurrent medications — NSAIDs reduce the antihypertensive efficacy of both Telmisartan and Hydrochlorothiazide; concurrent potassium-sparing diuretics or ACE inhibitors increase hyperkalaemia risk; lithium levels may be raised by diuretics
- Do not co-administer with Aliskiren in patients with diabetes or eGFR < 60 mL/min/1.73m² — dual RAAS blockade increases the risk of hyperkalaemia, hypotension, and renal failure
PCD Pharma Franchise for Telmisartan 40 mg + HCT 12.5 mg – Pharvax Biosciences
Pharvax Biosciences cordially invites pharma personnel, MRs, and distributors from all parts of India to come on board to work together on PCD Pharma Franchise and Monopoly Distribution rights for Telmisartan + Hydrochlorothiazide Tablets and the entire range of Medivaxia Pharma Cardio Diabetic Hypertensive range of products.
Hypertension afflicts more than 200 million Indians, making it the most common chronic cardiovascular disorder in India. Combinations of ARBs along with thiazide diuretics form one of the most commonly prescribed antihypertensive combinations in cardiology, internal medicine, and diabetology outpatient departments (OPDs). The 24-hour action, PPAR-γ activity, and cardiovascular outcomes from the ONTARGET study have made Telmisartan one of the most sought-after ARBs by cardiologists and diabetologists.
