Description

Amoxicillin and Potassium Clavulanate Injection I.P. is a broad-spectrum parenteral antibiotic combination that is produced and marketed by Pharvax Biosciences. It is an injection combination of Amoxicillin, which is a semi-synthetic aminopenicillin having a broad gram-positive and gram-negative spectrum of activity, along with Potassium Clavulanate, which acts as a beta-lactamase inhibitor and prevents enzymatic inactivation of Amoxicillin. The available strengths of this injection include 300 mg, 600 mg and 1.2 g vials. It is an evidence-based antibiotic used in moderate to severe bacterial infections in hospital and clinical settings.

This injectable form provides much higher and faster concentration levels of drugs in plasma and tissues than oral Amoxicillin and Clavulanate, and thus it is the first choice of administration when a fast bactericidal effect is clinically required – post-operative infections, intra-abdominal infections, severe pneumonia and sepsis treatment. This injection can be bought through Pharvax Biosciences’ Pharma PCD Pharma Franchise network.

Uses and Indications

Amoxicillin and Potassium Clavulanate Injection I.P. is prescribed by physicians, surgeons, and intensivists for the parenteral treatment of moderate to severe bacterial infections, including:

  • Lower respiratory tract infections – Hospital-acquired pneumonia (HAP), severe community-acquired pneumonia (CAP), Lung Abscess and Aspiration pneumonia needing IV treatment
  • Upper respiratory tract infections – Severe sinusitis, epiglottitis, and Parapharyngeal infections needing injectable antibiotics
  • Intra-abdominal infections – Peritonitis, Ruptured Appendicitis, Biliary Tract Infections and Other Polymicrobial Abdominal Sepsis
  • Urinary Tract Infections (UTIs) – Pyelonephritis, Complicated UTIs, Urosepsis needing parenteral treatment
  • Skin and soft tissue infections – Cellulitis, Diabetic Foot infections, Necrotising Fasciitis (in combination treatments) and Post-Operative Wound infections
  • Bone and Joint infections – Osteomyelitis and Septic Arthritis due to susceptible organisms
  • Obstetrical and Gynaecological Infections – Puerperal Sepsis, Endometritis, Pelvic Inflammatory Disease involving systemic involvement and Post Partum Infections
  • Septicaemia – Bloodstream infections due to Amoxicillin-Clavulanate susceptible organisms
  • Prophylactic use perioperatively – Surgical Site Infection prevention in Abdominal, Orthopaedic, and Gynaecological surgery
  • Ear, Nose and Throat infections – Severe Otitis Media, Mastoiditis, Deep Neck Space infections

How Amoxicillin + Clavulanate Injection Works

This injection delivers dual-mechanism antibacterial action combining broad-spectrum bactericidal activity with beta-lactamase protection — a combination that together achieves a clinically broader effective spectrum than unprotected Amoxicillin alone.

Amoxicillin is a broad-spectrum aminopenicillin whose bactericidal activity is achieved through binding to penicillin-binding proteins (PBPs), which occur on the inner surface of the bacterial cell wall; PBPs are the enzyme systems involved in the process of cross-linkage of peptidoglycan strands, which provide the bacterial cell wall’s strength. By competitively binding to PBPs (especially PBP1a, PBP1b and PBP2), Amoxicillin prevents the transpeptidation of the final step of the process of synthesis of the cell wall, which results in weakening of the cell wall, osmotic lysis and death of the bacterial cell.

Potassium Clavulanate is a naturally produced beta-lactam from Streptomyces clavuligerus, which serves as an irreversible and progressive inhibitor of bacterial beta-lactamases. Beta-lactamases are the main method by which bacteria develop resistance to penicillins, since they hydrolyse the beta-lactam ring of the antibiotic, rendering it ineffective before its interaction with the PBP target sites.

Side Effects

Amoxicillin and Potassium Clavulanate Injection is generally well tolerated when administered as directed. Reported side effects include:

  • Injection site reactions – pain, erythema, phlebitis at the injection site; more likely with higher concentrations or rapid injection
  • GI adverse reactions – nausea, vomiting, or diarrhoea; greater with IV than oral administration due to systemic drug exposure
  • Skin rashes and urticaria – skin symptoms indicative of allergy; could also be indicative of hypersensitivity
  • Liver enzyme elevations (AST/ALT) – usually reversible; Clavulanate accounts for increased hepatic enzymes in this drug combination
  • Cholestasis and jaundice – rare, with long-term treatment; common with the oral combination but can occur with the injection as well
  • Pseudomembranous colitis (C. difficile-associated diarrhoea) – occurs with broad-spectrum antibiotics; seek immediate medical care for persistent or bloody diarrhoea
  • Allergic/hypersensitivity reactions – range from mild rashes to severe anaphylaxis; more likely in those who have been allergic to penicillin
  • Haematological reactions – rare occurrence of thrombocytopenia, neutropenia, or haemolytic anaemia following long-term use
  • Superinfections – development of resistance to the drug or infections by fungi (such as Candida)

Precautions and Warnings

For hospital/clinical use only — reconstitution and administration should only be done by qualified health professionals under the direct supervision of the prescribing physician

Conduct hypersensitivity screening — ask for patient’s allergic reaction to penicillins, cephalosporins or any beta-lactam antibiotic before drug administration; have equipment for resuscitation and epinephrine ready

Assessment of renal function — test serum creatinine or eGFR before starting treatment; dosing should be modified in patients with CrCl less than 30 ml/min

Monitoring of liver function — conduct liver function tests in patients with a history of liver disease or in those on prolonged treatment

Not used in infectious mononucleosis (glandular fever) — the administration of amoxicillin in patients with mononucleosis caused by EBV leads to an increased risk of widespread skin rash; hence, it should not be used

Administration of intravenous infusion — infusion should be carried out slowly to avoid development of phlebitis; flush the intravenous infusion line before and after infusion to prevent precipitation of Amoxicillin with other drugs

Incompatible with other drugs — mixing with aminoglycosides such as Gentamicin or Amikacin is contraindicated due to incompatibility; both drugs will be inactivated when combined

Keep out of reach of children; treat as prescription injection after reconstitution
